6.      To qualify for assistance, you will need to provide the following supporting documents:


  1. For rental assistance
    • Applicants Photo ID (e.g., driver’s license, passport, or state ID)
    • Eviction, or late notice
    • Employment verification for adults in the household (If unemployed, provide most recent paystubs from prior employment, new hire letter, SSI, unemployment benefits, child support, etc.)
    • A fully signed lease agreement, or an Apartment Approval Letter from your landlord, if moving to a new apartment
    • Documents showing why you are unable to pay your rent or utilities this month (example paystubs showing less hours, etc.)
    • Proof you can sustain rent for the following month (example; new hire documentation, letter of pay increase, etc.)
    • Ledger or statement of late balance from your landlord

 

  1. For Utility Assistance
    1. Applicants Photo ID (example; driver’s license, passport, or state ID)
    2. Utility Bill or disconnect notice
    3. Employment verification for adults in the household (If unemployed, provide most recent paystubs from prior employment, new hire letter, SSI, unemployment benefits, child support, etc.)
    4. Proof you can pay for utilities the following month (e.g, new hire documentation, letter of pay increase, etc.)
